Step 1: Get a Referral

Ask your GP, treating specialist or hospital discharge planner for a written referral for DVA Community Nursing. This is required before care can begin.

Reach out by phone, WhatsApp or our contact form. If you're unsure where to start, we'll guide you through the referral process and what information DVA needs.

Step 3: Comprehensive Assessment

One of our Registered Nurses visits you at home to assess your clinical and personal care needs. We work with you, your family and your treating doctor to understand what support will help most.

Step 4: Care Begins

We develop a personalised care plan and start delivering the nursing and personal care you need, when you need it. We can also liaise with DVA on your behalf to confirm eligibility and entitlements, so you can focus on your health and wellbeing.
